[LIBRARY_NAME], “Subscriber”, has entered into an agreement with Counting Opinions (SQUIRE) Ltd, “Counting Opinions”, to provide an online Service for Reading Club Registration and Tracking of Reading Club Activity for participants enrolled in the Reading Club(s) offered through this Service.
This Service enables Registrants to register themselves and/or others as a Participant in the Reading Club(s) offered through this service. Subsequently both the Registrant and/or Participant can login and track Participant activity (Books read, Pages Read, Minutes Read, etc.) as well as submit Book Reviews. This information is not shared with anyone other than the Registrant who had registered the participant, the participant to whom the information is attributed, and the Subscriber or Counting Opinions staff who are authorized to access the information.
This Service also provides administrative functions for viewing and managing registrant and participant account details, awarding prizes, tracking fulfillment of loot and prizes, communication by email with previous consent, download of generic and personalized certificates and requesting/providing feedback.
Reporting features provide detailed and aggregate views of information collected. Detailed views can include personally identifiable information, whereas aggregate views include only generic groups and information and do not enable the differentiation of individuals or their personally identifiable information. Only users authorized by the Subscriber and those persons employed by Counting Opinions have access to personally identifiable information.

Information Collection and Use
Information Collected by the RC App
This service is highly configurable and may have variable requirements for personal and/or personally identifiable information (e.g., name, birthdate (or age), gender, address, phone, email, library card and PIN) for the Reading Club(s) offered through this service. Users always have the option of responding or not to any form or questions contained within the Service.
Counting Opinions on behalf of the Subscriber collects personal information when you decide to Register yourself and/or others the Reading Club(s) offered by the RCApp. This information may likely include:
If configured, you may register with the RC App using your Library Card and PIN (optional); name, birth date, email address, and address are automatically filled in the online form from your Library record. If some of this information is not available you may be required to provide the missing information. Otherwise you may opt-out of providing this information if not a requirement for Registration by the Subscriber.
Use of personal information
Personal information is used to create an RC App user account (Respondent), to provide the services/functions that are inherent to the RC App, monitor and improve these services, and customize and control content relevant to configured participant groupings (grouped by: age or grade, and/or gender).
If permitted by the Respondent, Name, School and Grade (if collected) may be used by the Subscriber to request records of grades from the School of attendance. This and the additional grade information will be used to analyze the impact that participating in a reading club has on the participants’ grades at school.
If permitted by the Respondent, email address may be used for the purpose of communication, notification, and invitation to participate in a feedback/evaluation program.
If permitted by the Respondent, names or usernames of participants may be posted and/or published in a Contest Winner list or announcement.
Children’s personal information
The RC App is designed and intended for use by children under the age of thirteen. Subscribers should have and maintain their own privacy policies related to the provision of these Services and their use and protection of children’s information. Counting Opinions does not use, sell, or trade any children’s’ information collected on behalf of the Subscriber and does not make this information accessible to any persons other than those with the appropriate authority.
Counting Opinions recommends that Respondents under 18 years of age should use the RC App offered by our Subscriber only with the involvement or permission of a parent or guardian.
Special Note for Children
Be sure to ask your parent or guardian for permission before sending any information about yourself to us (or anyone else) over the Internet.
Automated Information Collection
Counting Opinions automatically gathers certain information and stores it in log files. This information includes:
The data in this list has been marked as non-identifiable. This means that there is no reasonable way for Counting Opinions to identify the individual person this data was collected from.
Counting Opinions uses this information to analyze trends, to administer the website and system, to understand user movement within the Website, to improve quality of service, to aid solving technical issues, to support and enable the completion of current activity and for research and development.
This data will be used by Counting Opinions and their agents.
The Subscriber takes your privacy very seriously and does not sell, trade, or otherwise transfer to outside parties your personal information, except in the following limited circumstance:
Accessing, Changing or Deleting Information
Subscribers and Respondents may alter or delete information related to their accounts if not required for normal and trouble free functioning of the RC App (e.g., Age or Grade and/or Gender may be required for appropriate grouping and access to group appropriate content).
Respondents need to contact the Subscriber to request changes or deletions to information they have provided and do not have direct access. Subscribers have access to change or delete information in their data collections, but do not have the authority to delete feedback responses related to RC App feedback form. If requested by a Subscriber, Counting Opinions can change or delete a RC App feedback responses but may have to deny this request if compelled to do so by court order or other legal requirement.
Services configured to work in conjunction with the Subscriber’s ILS are asynchronous. As a result any changes and/or deletions made to information collected from the ILS are not synchronized with the ILS. These changes and/or deletions must be made directly on the ILS or communicated directly to the Subscriber to have the same change and/or deletion made.
Counting Operates dedicated and redundant Load Balancers (servers) running HAProxy and web application firewalls (WAF). The load balancers front-end the database servers and thus ensure that there is no direct access to any data. The load balancers are locked down and are not accessible by any Counting Opinions staff persons. Counting Opinions’ employees are not authorized to enter the server facility. Permission (from the principals of Counting Opinions) is required for any visit which requires an on-site escort and prior confirmed appointment. The server clusters is housed within buildings with multiple levels of secure access/entry with a minimum of guarded front-door, gated office entry and then secure access to server rooms, then secure access to servers.
Servers are monitored for intruder detection and distributed denial-of-service (DDOS), with alerts sent every 5-minutes until resolved. An application level lockout feature detects multiple consecutive failed login attempts (default 10) from the same user (source) and forces a temporary lockout from access by that IP (default 10 minutes). All requests to access servers and accesses to servers are logged and distributed via email and SMS notification.
Data updates are processed in real-time across our redundant database servers and eventually work themselves into the nightly backup cycle. Only authorized users have the ability to permanently dispose of data.
All forms have HTTPS access available to ensure a secure, encrypted communication between Counting Opinions servers and the user’s browser.
Changes to the Policy
Opting Out of Communications
Respondents have the right to opt out from receiving future communications, including no longer receiving email invitations to take surveys which are sent by the Subscriber and/or Counting Opinions on behalf of Subscribers. Respondents can contact the Subscriber (see “Governing Privacy Policies” section, above) to opt-out of receiving any further communications.

The website may contain links to other websites that are not owned or controlled by the Subscriber or Counting Opinions. Please be aware that the Subscriber and Counting Opinions cannot control the activities of third parties and the Subscriber and Counting Opinions do not accept any responsibility for the privacy practices of such other websites. The Subscriber and Counting Opinions encourage persons to be aware when they leave the RC App and to read the privacy policies of each and every website that collects personally identifiable information.
